AHF Pharmacy has a clinical team of pharmacists and nurses called Integrated Medication Management (IMM). Our IMM team will work with you and your healthcare provider to make sure that your medications are appropriate, safe, and effective through a yearly medication review. This service is available at no extra cost to you.

Through This Service, Our IMM Team Will:

Review your health conditions and medications, including prescription and over-the-counter medications, vitamins, and supplements.
Round out your care by not only discussing your medications, but reviewing healthy lifestyle changes and your immunization history.
Provide helpful tips to reduce side effects and address any additional concerns you may have about medications.
Communicate and work with your healthcare providers to optimize your medications.
Provide you with a written summary of your medication review to share with your doctors and health care team.

How To Participate

The review takes between 15-45 minutes. You may receive a call from one of our IMM team members. Or, you can call the IMM team to schedule or complete your medication review at a time that is convenient for you. If you have questions and want to learn more about our program, please contact us.
